Trial sessions are by appointment only, so it is important that the below form is completed and payment made before attending a trial session. You need to apply no less than 5 days before the session you wish to attend.
All details are of the proposed member so a parent or guardian must input for under 16s (if you are a family, please submit one form for each of you).The disclosure of medical issues is of paramount importance to ensure we are able to keep you and other members safe.
